Independent Services At SmartLife

The goal of SmartLife VIA Willow Valley is to keep you well, independent, and in your home. However, if staying at home ever becomes unsafe, personal care, memory support care, and nursing care will be provided for SmartLife Members at Willow Valley Communities and at other plan-participating providers. 

Personal Care will be provided when a SmartLife Member needs ongoing assistance with activities of daily living (ADL’s) such as eating, bathing, dressing, grooming, transferring, toileting, and walking. Your Lifecare Coordinator will take care of personal care coordination.

Nursing Care will be provided to SmartLife Members when they need short-term rehabilitation services to recover from an illness or surgery, such as a knee replacement. Long-term, around-the-clock nursing care will be provided when you can no longer stay in your home or in a personal care setting. Your Lifecare Coordinator will take care of nursing care coordination.

Memory Support Care will be provided to SmartLife Members when they need enhanced safety due to memory issues. Your Lifecare Coordinator will take care of memory support coordination.
