We deliver services in your home

No one is ever prepared for a life changing event. It can be so overwhelming. Where do you go for help? If you are a SmartLife Member, you would simply pick-up the phone and call your Lifecare Coordinator. Your Lifecare Coordinator will then put services into place for you.

Your Lifecare Coordinator will work closely with you, your family and your physician to have the right services delivered to you so you can keep an independent lifestyle at home or when necessary, in personal care or skilled nursing care at Willow Valley Communities or other plan-participating providers. A Lifecare Coordinator will be available to you, 24-hours-a-day, 7-days-a-week.

Some of the in home services for seniors we offer, based on a medical need, are:

Home Care Aides will provide in home senior care and support when you have a decline in as little as one activity of daily living (ADL). ADL’s include eating, bathing, dressing, grooming, transferring, toileting, and walking.

Companion/Homemaker will be provided when you need assistance in your home such as: supervision of activities of daily living (ADL’s) and/or medication reminders. This home care companion can also do cooking, dishwashing, laundry, light housekeeping, and errands while they are present in your home for eligible service.

Home Safety Assessment will be conducted during your first year as a SmartLife Member, and every second year thereafter by a Licensed Occupational Therapist to keep you safe in your home now and into the future.

Transportation will be provided for SmartLife Members to and from medically necessary appointments, outpatient procedures, and surgeries when you are not permitted to drive.

Emergency Response System will be provided, as needed, with 24-hour coverage.

Referral Services are a fee-for-service arrangement that links SmartLife Members to any type of service you may need to stay in your home. Examples of these services are legal, financial planning, home maintenance, rental of medical equipment, and more. You will be responsible for paying for these services and may receive a discount as a SmartLife Member.

Meals including lunch and dinner can be delivered to your home when you are unable to cook due to a short-term illness.
