Meet with a SmartLife Advisor

We understand that applying for Membership with SmartLife VIA Willow Valley is an important decision that needs careful consideration. One of the first steps in the process is to meet with a SmartLife Advisor. The Advisor will share more about the program, answer your questions about in home care, review plan pricing, and assist you in determining if SmartLife is the right choice for you.

To apply for SmartLife VIA Willow Valley Membership, you must complete a Membership application, a financial application and have a one-on-one meeting with the Lifecare Coordinator. We will also require three years of medical history from your family physician.

There is a non-refundable $150.00 application fee. The minimum age to join SmartLife is 60. There is no maximum age.
