As a Member, you are assigned a Lifecare Coordinator who will assist you and your family in navigating the time consuming challenges of arranging for long-term care. There are no forms to process or puzzling papers to complete—just one simple call to your Lifecare Coordinator will initiate care when you need it.

Your Lifecare Coordinator will work closely with you, your family, and your physician to have the right services delivered to you in your home, or when necessary, at providers in your local area. A member of the Care Coordination Team will be available to you, 24-hours a day, 7 days a week.
